Establishing a Community-Based DSME Program for Adults with Type 2 Diabetes to Improve Glycemic ControlThis Action Guide provides information on the resources and key steps to establish a community-based diabetes self-management education program to improve glycemic control among adults with type 2 diabetes. Community-based diabetes self-management education is effective in improving glycemic control among adults with type 2 diabetes. It can be conducted in community gathering places such as community centers, libraries, private facilities, and faith-based institutions. With an emphasis on convenient locations, community support, and cultural relevance to participants, community-based DSME is especially important for reaching people who have limited access to formal healthcare, do not speak English, or may not have the option of clinic-, home-, or worksite-based diabetes education. It is typically delivered by health professionals and public health practitioners and should include coordination with an individual’s primary care provider and any diabetes education received in a clinical setting.
